Your MRI May Not Explain Your Back Pain: What the Research Actually Shows

If you've ever had an MRI for back pain, you've probably seen words that sound alarming:
Disc bulge.
Degeneration.
Arthritis.
Disc protrusion.
Facet joint changes.
For many people, reading an MRI report triggers a wave of concern.
Questions start racing through their mind:
Is my spine damaged?
Should I stop exercising?
Am I making this worse every time I bend over?
Will this continue to get worse?
But what if some of the findings that sound alarming are actually quite typical?
Research over the past several decades has revealed something that surprises both patients and healthcare providers:
Many people with no back pain at all have the same MRI findings commonly blamed for back pain.
The Study That Changed How We Think About Back Pain
In a landmark review published in The American Journal of Neuroradiology, researchers examined imaging studies of people who had no back pain.
The results were eye-opening.
Among people with no symptoms:
Disc degeneration was common.
Disc bulges were common.
Disc protrusions were common.
Arthritic changes were common.
Perhaps most surprising, these findings became more common with age.
By age 50, the majority of pain-free adults showed some degree of disc degeneration on imaging.
By age 60 and beyond, imaging changes that sound concerning were often the norm rather than the exception.
In other words:
Many MRI findings may reflect age-related changes rather than a specific cause of pain.
These findings have been replicated repeatedly. In the widely cited 2015 systematic review, Systematic Literature Review of Imaging Features of Spinal Degeneration in Asymptomatic Populations, researchers analyzed imaging findings from more than 3,000 asymptomatic individuals and found that disc degeneration, disc bulges, and other spinal changes were extremely common even among adults who reported no pain. The prevalence of these findings increased steadily with age, reinforcing the idea that many MRI findings may reflect typical age-related changes rather than a direct cause of symptoms.
At this point, you may be thinking:
"That's interesting, but I do have back pain."
And that's an important distinction.
The research is not suggesting that your pain isn't real.
Nor does it suggest that MRI findings are meaningless.
Instead, it highlights a surprising fact:
Many of the structural changes seen on imaging are also found in people who have no symptoms.
This means that while an MRI may identify changes within the spine, it does not automatically tell us which findings are responsible for a person's pain—or whether they are responsible at all.
That distinction has become one of the most important discoveries in modern back pain research.
Wait... Then Why Does My Back Hurt?
This is where things become interesting.
Modern pain science has shown that pain is rarely explained by a single factor.
Back pain can be influenced by:
movement habits
muscular endurance
stress levels
sleep quality

nervous system sensitivity
This does not mean pain is "all in your head."
Pain is real.
But it does mean that the relationship between pain and imaging findings is often much more complicated than people assume.
The MRI Trap
One of the challenges with imaging is that people naturally assume abnormal findings equal damage.
Imagine reading:
"Degenerative disc disease."
That sounds serious.
Yet many healthcare professionals now point out that this phrase often describes typical age-related changes rather than a disease process in the traditional sense.
Hair turns gray.
Skin develops wrinkles.
Spinal structures change with age as well.
Many of these changes are visible on imaging long before they ever cause symptoms—if they cause symptoms at all.
Why This Matters
Research has shown that beliefs about pain can influence how people move.
After receiving an MRI report, many people begin changing their behavior in ways they barely notice.
They may hold tension through their torso constantly.
They may stiffen their back when standing or walking.
They may stop bending, twisting, reaching, or getting down to the floor.
Some begin avoiding activities they previously enjoyed because they fear causing further damage.
Over time, the body can become less adaptable to movement.
Muscles remain tense.
Movement becomes less natural.
Confidence decreases.
Ironically, many people begin moving as though their spine is fragile when, in reality, the body is often far more resilient than they realize.
For some individuals, this protective response may contribute to stiffness, reduced mobility, and increased sensitivity over time.
What the Research Suggests

"What's wrong with my spine?"
A more helpful question is often:
"What actually matters for long-term function?"
Modern approaches to back pain increasingly focus on restoring movement, confidence, strength, mobility, and function rather than simply chasing imaging findings.
For many adults, learning how to move comfortably again becomes far more important than trying to achieve a "perfect" MRI.
