days from date of purchase. The new client special grants you access to all regularly scheduled
adult live and live-streamed classes.

90 Days of Unlimited Yoga: Our 90 day package gives you truly unlimited access to all regularly
scheduled live and live streamed adult yoga classes. You may choose to pay for your
membership upfront or utilize our payment plan that allows you to pay for this package over 3
months. There is no extra charge for using a payment plan. Total cost of this plan is $525.

12 Month Unlimited Yoga Contract: When purchasing this contract, you are agreeing to
complete the full term of the contract. This contract can be paid in full on the date of purchase. A
payment plan is available for this contract. By making this purchase you are giving Foundations
Yoga and Therapy LLC to charge the payment method that you have on file for all seven
payments that are included with the payment plan option.

8 Classes per Month (auto-renew) membership: Our 8 class Monthly Membership is $162 per
month and will automatically be charged to the payment method that you have on file every
month unless you cancel your membership through email. There is no commitment. You can
cancel at any time with a 14 day notice by email. By making this purchase you are giving
Foundations Yoga and Therapy LLC permission to charge your card $162 each month. Classes
do not roll over. If you don't use your 8 classes in the month you will lose them.

15 Class Pass: The 15 class package is valid for regularly scheduled live and online adult group
yoga classes. This package expires 6 months from the date of purchase. Expiration dates are
firm. Package expiration dates are not eligible to be extended.

Attendance: Group classes - Pre-registration is required for all classes. Should your plans
change, class registrations must be canceled no later than two hours before class start time in
order to avoid a late cancellation/ no show fee. Class packages will be charged in full. Unlimited
contract holders will be charged a $10 fee for late cancellations and no shows.

Private session services: All private session services must be pre-booked. Private session fees
are non refundable and non transferable. Sessions that are canceled with less than 24 hours
notice are not available to be rescheduled. Single private sessions are valid for three months
from the date of purchase. Select services offer 3 session package pricing. Packages are valid
for four months from the date of purchase. For clients who have regularly scheduled
appointments, attendance requirements include maintenance of an 80% attendance rate
measured over any given three month period.
