I hereby acknowledge that my child is participating in yoga classes and activities at Foundations
Yoga and Therapy, LLC in Madison, NJ. “Claims” includes but is not limited to any and all
liabilities, claims, demands, legal actions, rights of actions for damages, personal injury or death
in connection with participation in yoga and yoga related activities. “Released Party” means
Foundations Yoga and Therapy, LLC or any of its affiliates, employees, representatives, or
volunteer staff.

I understand that there are inherent risks involved in yoga practice, including but
not limited to physical exertion, stretching, and the potential for injury. I certify that my child is
physically capable of participating in yoga classes, and I take full responsibility for their health
and well-being during these activities. In consideration of my child being permitted to participate
in yoga classes at Foundations Yoga and Therapy, LLC I hereby release, waive, and discharge
all claims against the released party from any and all liability for personal injury, including death,
or property damage that may arise out of my child's participation in these activities.

I agree not to sue any Released Party for Claims, even if the Claims arise from the carelessness,
negligence or gross negligence of any Released Party or anyone else. I agree to indemnify
(reimburse for any loss) and hold harmless each Released Party from any loss or liability
(including any reasonable legal fees they may incur) defending any Claim made by me or
anyone making a Claim on my behalf, even if the Claim is alleged to or did result from the
carelessness or negligence of any Released Party or anyone else. I understand that it is my
responsibility to inform the instructor of any physical limitations or medical conditions my child
may have prior to participating in yoga classes. I have read this waiver and fully understand its
contents. I am aware that by agreeing to this waiver, I am waiving certain legal rights on behalf
of myself and my child.

By agreeing to this waiver, I acknowledge that I have read, understand,
and agree to all terms and conditions outlined herein.
